A Nurse Case Manager with a Certified Nursing Assistant or another Licensed Professional will conduct a comprehensive assessment of the client either at the Nursing Facility or in the client’s home. The evaluation will cover the following key areas: – This process ensures compliance with state regulations that mandate services to be provided by a licensed home health agency. – Medical Records are securely maintained at the HHA Office and the Client’s Residence. – Assessment of the client’s situation and care need on a daily, weekly, or monthly basis. – Thorough review of the client’s medical records, medical history, diagnosis, dietary requirements, medications, treatments, and necessary equipment. – Evaluation of the client’s functional ability, skin condition, required supplies, and level of assistance needed. – Identification of personal preferences, daily routines, available resources, and support system. – Warm Towel / Soap / Lotion – Care – Personal hygiene assistance – Toileting and incontinence care – Assisting with dressing – Safely transferring from bed to chair – Skin observation – Application of skin protectant cream – Monitoring vital signs such as blood pressure, pulse, temperature, SPO2, and pain – Assistance with Meal

We provide a comprehensive solution customized to meet your demanding schedule. Establish a professional relationship and collaborate with a dedicated Care Coordinator who will oversee all your healthcare and wellness needs. The Care Coordinator will efficiently coordinate and manage required needs in collaboration with the client and Power of Attorney (POA) for the upcoming two weeks or longer. Bi-weekly calls or calls as required are included. This involves arranging and scheduling transportation, medical appointments, procedures, and diagnostics, as well as managing services, pharmacy needs, supplies, and household essentials. A detailed summary and cost estimate will be prepared for approval by the POA before any actions are taken. Please be advised that all expenses related to the client’s needs will be the responsibility of the client or the designated POA. Allocating 15 to 30 minutes to discuss your needs will enable our Care Navigators to promptly address your requirements. Ask for pricing
